National Care Dental has multiple coverage options that will work for you and your family. The following plans are available.
National Care Dental - up to $1,500 annual benefit
National Care Dental - up to $3,000 annual benefit
National Care Dental - up to $5,000 annual benefit
National Care Dental is underwritten by Nationwide Life Insurance Company through the National Small Business Association (NSBA). NSBA uses the Maximum Care PPO Plus Connection Dental Network, which includes preferred provider networks such as Careington, DenteMax and Connection Dental networks. VSP Vision options are also available with National Care Dental plans.

Preventive Services
Type I – 100% Coverage
No waiting period. No Deductible.
Includes oral exams, cleanings, bitewing x-rays, and fluoride treatment.
Basic Services
Type II – 80% Coverage
No waiting period. $50.00 Individual deductible.
Maximum Deductible per Family: 3 times the Individual
Includes fillings, full mouth x-rays, restorative amalgams, composites, simple extractions, and sealants.
Major Services
Type III – 50% Coverage
12 month waiting period. $50.00 Individual deductible.
Maximum Deductible per Family: 3 times the Individual
Includes oral surgery, endodontics, complex extractions, periodontics, onlays, anesthesia, dentures, crowns, bridges and implants

Delta Dental PPOTM
Delta Dental PPOTM is a coinsurance plan that gives enrollees the freedom to visit any licensed dentist, but they’ll usually get the deepest discounts by visiting in-network dentists. Outside of the low-cost PPO network, enrollees may also enjoy moderate savings with the Delta Dental Premier network.
